Whether you or your loved one is moving from the hospital to Bethesda, or from Bethesda back home, transition counseling at Bethesda Rehab & Senior Care is here to help.
Our licensed social worker (LSW) attends each resident's care plan meetings during their stay, and coordinates discharge planning to ensure you have the equipment and services you need so that your transition is a painless process. Bethesda's social workers help to treat stress, depression, and other psychological concerns many residents may experience as a result of their medical conditions. Our expert counseling in combination with other therapies can significantly improve your recovery.
Our staff is always on hand to assist you with all aspects of the transitions of care, including:
- Pre-admission assessment and planning
- Supportive counseling and support
- Care conference coordination
- Discharge planning
- Assistance with arranging home care services
- Hospice and palliative care
- Long-term care insurance
Your recovery doesn’t end when you leave Bethesda- we know that getting back to your full function is an ongoing process that you will continue when you return home. To help with this transition, our highly qualified team of care professionals, licensed social workers, and therapists make sure that you are ready for the challenges of completing your recovery at home.
Our licensed social workers coordinate the discharge planning process. As a part of our discharge planning services, our staff will work with you to assess your home environment and ensure that you have all of the proper equipment to meet your needs. Before you leave Bethesda, our therapists create a personalized plan to continue your recovery at an alternate level of care.
